A pill you give at home, or a shot at the clinic. That's the real Numelvi vs Cytopoint question. Here's how I'd put them next to each other in the exam room.

The side-effect numbers come straight from each drug's label, and the Cytopoint prices are the rough 2026 US figures already on the itch-meds guide. Numelvi still has no price I'd trust to quote.

Numelvi (atinvicitinib) is a once-a-day JAK pill. Cytopoint (lokivetmab) is a clinic shot that grabs IL-31, the main itch signal, before it reaches the nerve. They stop the same itch in two different places: Numelvi inside the cell, Cytopoint outside it.

None of these are over the counter. Every one needs a prescription from a vet who has examined your dog.

One honest warning before you compare side-effect numbers between drugs: they come from different studies of different lengths, from about two weeks to four months. A drug studied for four months will rack up more problems on paper than one studied for 28 days. Even the USDA's own Cytopoint summary says not to use these studies to compare one product to another.

What it does, and what's different

Numelvi (atinvicitinib) is the newest. It's a once-a-day pill given with food, and it's more targeted than the older JAK drugs: it's built to block mainly one enzyme, JAK1. It's labeled for dogs as young as 6 months, younger than Apoquel or Zenrelia, and it has no vaccine warning; in its vaccine study, dogs on Numelvi responded normally to their shots. The catch is that it's new. Its field study ran only 28 days, and the label says it hasn't been studied in the field beyond that. I couldn't find a price I'd trust yet, so ask your vet's office.

Real side effects

  • In its 28-day study (144 dogs on Numelvi, 144 on placebo): vomiting or nausea 6.9% vs 4.2%, ear infections 6.3% vs 5.6%, blood in the urine 4.9% vs 4.2%, poor appetite 4.2% vs 3.5%. Diarrhea was actually more common on placebo (10.4%) than on Numelvi (4.2%).
  • Not for dogs under 6 months or dogs with serious infections. It may make infections more likely, including demodex mange and infected cysts between the toes, and new tumors have been reported in dogs on other JAK drugs.

What it does

Cytopoint (lokivetmab) is an injection, not a pill. It's a lab-made dog antibody that grabs IL-31, the "itch cytokine" that drives so much of the itch in dogs, and takes it out of play before it can reach the nerves. It works outside the cells, not inside them like the JAK pills, and Zoetis says it has minimal impact on the rest of the immune system. Your vet gives it in the clinic.

How fast, and how long it lasts

Zoetis says one shot starts controlling itch within a day, and the 2023 AAHA guidelines put the onset anywhere from hours to 3 days. One injection lasts 4 to 8 weeks, and it's repeated every 4 to 8 weeks as needed, depending on the dog.

Rough cost

Cytopoint is priced by weight and only given at the clinic. Figure about $120 to $200 a shot for a 55 lb dog, from about $50 for small dogs and $200 to $300 or more for giant ones. At one shot every 8 weeks that's about $800 to $1,300 a year. These are rough 2026 US prices, and your clinic will vary.

Who it suits

Cytopoint has the fewest strings attached of anything on this page. The 2023 AAHA guidelines list no age limit and no health restrictions for it, and call it safe for puppies and for dogs with other illnesses, including cancer. In the US safety study, dogs on Cytopoint were also taking flea and tick products, antibiotics, steroids, vaccines and even Apoquel at the same time.

Real side effects

  • In the US field safety study (162 dogs on Cytopoint, 83 on placebo), the only problems were at the injection site: about 6 in 100 dogs seemed sore after the first shot, versus about 5 in 100 on placebo. No allergic reactions to the shot were seen.
  • Rare, 1 to 10 dogs in every 10,000 treated (from the UK label's reporting data): allergic reactions such as facial swelling, hives or anaphylaxis; vomiting or diarrhea; and neurologic signs such as wobbliness or seizures.
  • Very rare, under 1 in 10,000: immune diseases where the body attacks its own red blood cells or platelets.
  • Uncommonly, a dog's immune system makes antibodies against Cytopoint itself, and the shots stop working as well for that dog.

The full Cytopoint write-up is on itch meds, honestly.

So which one?

If you're wondering where I usually start: for a dog that needs help most of the year, Apoquel or Cytopoint, because both work fast and their side effects are usually mild. Cytopoint is my go-to for puppies, older dogs and dogs with other health problems. Steroids are for short bursts. And allergy shots are the long game for the dog who'd otherwise be on medicine for life.

Numelvi is the newest JAK pill. Unless your dog needs it, I like to let other people's dogs go first for a year or two. I couldn't find a price I'd trust yet, so ask your vet's office.

Comparing the two JAK pills instead? That's Numelvi vs Apoquel. Zenrelia, steroids and allergy shots live on the full itch-meds guide.

Questions I get every day

The short answers, in plain words. Each one points to the part of the page that goes deeper. More questions and answers.

Numelvi vs Cytopoint: what's the difference?

They stop the same itch in two different places. Numelvi is a once-a-day JAK pill given with food, built to block mainly JAK1 inside the cell, labeled for dogs as young as 6 months. Cytopoint is a shot at the clinic every 4 to 8 weeks that grabs IL-31 before it reaches the nerve, and the 2023 AAHA guidelines list no age limit. Numelvi is new, with a 28-day field study and no price I'd trust yet. Cytopoint is about $120 to $200 a shot for a 55-pound dog.

The side-by-side table

Which is better for dogs, Numelvi or Cytopoint?

Neither is better for every dog. Cytopoint is my go-to for puppies, older dogs and dogs with other health problems, including cancer. If a dog needs help most of the year, I usually start with Apoquel or Cytopoint. Numelvi is the newest JAK pill, approved in early 2026. Unless your dog needs it, I like to let other people's dogs go first for a year or two.

So which one?

Is Numelvi cheaper than Cytopoint?

I couldn't find a price I'd trust for Numelvi yet, so ask your vet's office. Cytopoint is priced by weight and only given at the clinic: about $120 to $200 a shot for a 55-pound dog, from about $50 for small dogs and $200 to $300 or more for giant ones. At one shot every 8 weeks that's about $800 to $1,300 a year at rough 2026 US prices.

The side-by-side table

Can puppies take Numelvi or Cytopoint?

Yes, with different age floors. Cytopoint has no age limit in the 2023 AAHA guidelines, and they call it safe for puppies. Numelvi is labeled for dogs as young as 6 months. Both need a vet: Cytopoint is a clinic shot, and Numelvi is a prescription pill. If your dog has other illnesses, including cancer, Cytopoint is the one the guidelines call safe in that setting.

Cytopoint, in full

Can my dog get vaccines on Numelvi or Cytopoint?

Yes. Numelvi has no vaccine warning, and in its vaccine study, dogs on Numelvi responded normally to their shots. Cytopoint isn't a JAK pill: in the US safety study, dogs on Cytopoint were also taking vaccines at the same time, along with flea and tick products, antibiotics, steroids and even Apoquel. That's different from Zenrelia, which has a boxed vaccine warning and a stop-and-wait rule around shot day.
