If your vet mentioned Numelvi, the next question is almost always Apoquel. Same family of pills, two different labels. Here's the side-by-side I'd walk through in the exam room.
The side-effect numbers come straight from each drug's label, and the Apoquel prices are the rough 2026 US figures already on the itch-meds guide. Numelvi still has no price I'd trust to quote.
Numelvi (atinvicitinib) and Apoquel (oclacitinib) are both JAK-blocking pills for allergic itch. They work the same basic way, blocking the itch signal inside the cell, but each has its own label, and the labels aren't the same. Apoquel came out in 2013. Numelvi, from Merck Animal Health, was approved in early 2026.
None of these are over the counter. Every one needs a prescription from a vet who has examined your dog.
At a glance
| Numelvi | Apoquel | |
|---|---|---|
| Form | A once-a-day pill, given with food | A pill, or a chewable since 2023 |
| Dosing | Once a day | Twice a day for up to two weeks, then once a day, with or without food |
| Youngest age | 6 months | 12 months |
| Onset | No onset time I'd trust to quote yet | As little as 4 hours, Zoetis says |
| Vaccine warning | None. Dogs on it responded normally to their shots | No vaccine timing rule quoted |
| Cost | No price I'd trust yet. Ask your vet. | Roughly $80 to $110 a month for a 55 lb dog (first month about $120 to $140) |
One honest warning before you compare side-effect numbers between drugs: they come from different studies of different lengths, from about two weeks to four months. A drug studied for four months will rack up more problems on paper than one studied for 28 days. Even the USDA's own Cytopoint summary says not to use these studies to compare one product to another.
Numelvi

- Form
- A once-a-day pill, given with food
- How often
- Once a day
- Youngest age
- 6 months
What it does, and what's different
Numelvi (atinvicitinib) is the newest. It's a once-a-day pill given with food, and it's more targeted than the older JAK drugs: it's built to block mainly one enzyme, JAK1. It's labeled for dogs as young as 6 months, younger than Apoquel or Zenrelia, and it has no vaccine warning; in its vaccine study, dogs on Numelvi responded normally to their shots. The catch is that it's new. Its field study ran only 28 days, and the label says it hasn't been studied in the field beyond that. I couldn't find a price I'd trust yet, so ask your vet's office.
Real side effects
- In its 28-day study (144 dogs on Numelvi, 144 on placebo): vomiting or nausea 6.9% vs 4.2%, ear infections 6.3% vs 5.6%, blood in the urine 4.9% vs 4.2%, poor appetite 4.2% vs 3.5%. Diarrhea was actually more common on placebo (10.4%) than on Numelvi (4.2%).
- Not for dogs under 6 months or dogs with serious infections. It may make infections more likely, including demodex mange and infected cysts between the toes, and new tumors have been reported in dogs on other JAK drugs.
Apoquel

- Form
- A pill, or a chewable
- How often
- Twice a day for up to two weeks, then once a day
- How fast
- As little as 4 hours, Zoetis says
What it does
Apoquel (oclacitinib) is a pill, and since 2023 there's a chewable version too. The itch messengers in allergic skin, including IL-31, the main itch signal in dogs, pass their message along inside the cell using enzymes called JAKs. Apoquel blocks those enzymes, so the message doesn't get through. It's not a steroid and it's not an antihistamine. It came out in 2013 and changed how vets treat itchy dogs.
How fast, and how it's given
It works fast. Zoetis says it starts relieving itch in as little as 4 hours, and in the label's one-week study, owners rated dogs on Apoquel less itchy than dogs on placebo on every one of the first 7 days. It's given twice a day for up to two weeks, then once a day after that, with or without food.
Rough cost
Figure roughly $80 to $110 a month for a 55 lb dog, and about $120 to $140 for the first month while it's given twice a day. That's about $960 to $1,320 a year. These are rough 2026 US prices, and your clinic and city will vary, so ask for a quote.
Who shouldn't take it
The label says Apoquel isn't for dogs under 12 months old, dogs with serious infections, or breeding, pregnant or nursing dogs. Because it changes how the immune system works, it can make a dog more prone to infections, including demodex mange, and it can make existing tumors worse. New lumps and cancers, both benign and malignant, showed up in dogs during the studies. The best long-term comparison, about 660 allergic dogs, found no difference in tumors between dogs on Apoquel and dogs on other treatments. Still, if your dog has had cancer or keeps getting serious infections, that's a real conversation to have with your vet before starting it.
Real side effects
- First 16 days of the label's atopic dermatitis study (152 dogs on Apoquel, 147 on placebo): diarrhea 4.6% vs 3.4%, vomiting 3.9% vs 4.1%, poor appetite 2.6% vs 0%, lethargy 2.0% vs 1.4%. Most of it settled down on its own while the dogs kept taking the pill.
- Counting everyone who got Apoquel in that study and its open follow-up (283 dogs), the most common new problems were skin infections (12%), lumps of various kinds (12%), ear infections (about 10%) and vomiting (about 9%). There was no placebo group to compare against for those numbers, so it's hard to say how much was the drug and how much was the allergy itself.
- In a follow-on study, 239 dogs stayed on Apoquel for an average of about a year. A small number developed tumors, including 6 dogs put to sleep for suspected cancers, and there was no comparison group. That's why the label tells vets to watch dogs on Apoquel for infections and new lumps.
The full Apoquel write-up, including what to do if it wears off before the next dose, is on itch meds, honestly.
So which one?
If you're wondering where I usually start: for a dog that needs help most of the year, Apoquel or Cytopoint, because both work fast and their side effects are usually mild. Cytopoint is my go-to for puppies, older dogs and dogs with other health problems. Steroids are for short bursts. And allergy shots are the long game for the dog who'd otherwise be on medicine for life.
Numelvi is the newest JAK pill. Unless your dog needs it, I like to let other people's dogs go first for a year or two. I couldn't find a price I'd trust yet, so ask your vet's office.
Comparing Numelvi with the clinic shot instead? That's Numelvi vs Cytopoint. Zenrelia, steroids and allergy shots live on the full itch-meds guide.
Questions I get every day
The short answers, in plain words. Each one points to the part of the page that goes deeper. More questions and answers.
Numelvi vs Apoquel: what's the difference?
Both are JAK-blocking pills for allergic itch, but the labels aren't the same. Numelvi is a once-a-day pill given with food, built to block mainly JAK1, labeled for dogs as young as 6 months, with no vaccine warning. Apoquel is a pill, with a chewable version since 2023, given twice a day for up to two weeks then once a day, and it isn't for dogs under 12 months. Numelvi is new: its field study ran only 28 days, and I couldn't find a price I'd trust yet.
Which is better for dogs, Numelvi or Apoquel?
Neither is better for every dog. They stop the itch the same basic way, by blocking JAK enzymes inside the cell, but Apoquel has been around since 2013 and Numelvi was approved in early 2026. I usually start with Apoquel or Cytopoint for a dog that needs help most of the year. Unless your dog needs Numelvi, I like to let other people's dogs go first for a year or two. Talk it through with your vet.
Is Numelvi cheaper than Apoquel?
I couldn't find a price I'd trust for Numelvi yet, so ask your vet's office. Apoquel is about $80 to $110 a month for a 55-pound dog, and about $120 to $140 for the first month while it's given twice a day, or about $960 to $1,320 a year at rough 2026 US prices. Your clinic and city will vary, so ask for a quote.
Can a 6-month-old puppy take Numelvi instead of Apoquel?
Numelvi is labeled for dogs as young as 6 months. Apoquel isn't for dogs under 12 months. Cytopoint has no age limit in the 2023 AAHA guidelines, and they call it safe for puppies. None of these is over the counter. Every one needs a prescription from a vet who has examined your dog, and Cytopoint is only given at the clinic.
Does Numelvi have a vaccine warning?
No. Numelvi has no vaccine warning, and in its vaccine study, dogs on Numelvi responded normally to their shots. That's different from Zenrelia, which carries a boxed warning about vaccines and a stop-and-wait rule around shot day. Apoquel isn't described with that vaccine timing rule either.
